Exercise and ADHD
Why it matters, why it's hard, and how to make it easier
Many ADHDers love exercise, particularly cardiovascular exercise, which is fantastic. It’s one of the best natural ways to flood the prefrontal cortex with the dopamine and noradrenaline it needs to function well — making it easier to focus, learn, remember, regulate emotions, and manage impulses. But for some of us, while we may be strong, ask us to do anything that requires even brief cardiovascular endurance and our hearts sink, because we struggle to do it. This can have a massive impact on our attitude and desire to exercise.
If this is you and you’ve always wondered what’s ‘wrong’ with you, it’s probably not a you problem. It’s an ADHD problem.
The ‘missing’ endurance problem in ADHD
You know that feeling when you’re walking uphill with everyone else, working twice as hard, and they’re chatting away like it’s nothing while you’re sounding like a broken ventilator, your muscles on fire. Your body is behaving as if you’ve never exercise.
But you’re not unfit. You exercise. You’re strong. You can lift, push, sprint. But when you need the sustained effort needed climb a hill or set a faster pace, your body seems to hit a ceiling that theirs doesn’t seem to. It upsets you. And it certainly colours your perception of exercise. You loathe it.
This is one of the least talked about experiences in ADHD, and one of the most demoralising. You may have high explosive strength, but your ceiling for aerobic endurance may be low to zero. Why?
The power plant (Mitochondrial dysfunction or inefficiency)
Every cell in your body is filled with tiny power plants called mitochondria. These turn the food you eat and the air you breathe into the fuel your muscles need to function. Some research suggests that in ADHD, theses mitochondria may work less efficiently during endurance exercise. You have plenty of ‘explosive’ power. A brief sprint or lifting something heavy feels easy, but as soon as need endurance, the power plants start to fail.
The dopamine-energy connection (the ‘central fatigue’ glitch)
We usually think of dopamine as the attention chemical, but it also acts as the brain’s energy manager, influencing motivation and willingness to sustain effort.
In the ADHD brain, where dopamine is in short supply, that decision-making system errs on the safe side because it perceives sustained effort as far more costly than it actually is. So, when it senses the effort, it pulls the handbrake too early, making us feel as if we have nothing left to give.
This throttle effect is real, and it is being applied by our own brain, not because our muscles are empty, but because the signal to empty them is being held back. We hit the wall because our brain saw orange, and responded as if it was red.
The oxygen delivery gap (iron issues)
The next issue... To keep your mitochondria operating, your blood must carry oxygen. This needs iron (specifically stored iron called ferritin). Unfortunately, ADHD bodies are known to struggle with low iron so, if your stores are low, your muscles will be essentially gasping for air. It’s another reason why you may hit that ceiling so quickly.
Chronic ‘autonomic’ drain (perpetual fight or flight)
Finally, many ADHDers live in a state of low-grade fight or flight, which means just to exist, we burn through the cellular resources — magnesium, B vitamins, zinc— needed to power us, so when we need them to power us up a mountain, they aren’t always there.
Please note: None of this is definitive. The research is still evolving and not everyone with ADHD experiences this, but if you’ve spent years feeling like your body wasn’t on your side during exercise, this could be why.
What you feed your body with, and how, helps.
How you exercise helps. Time, effort and duration, done properly, can build more of those little energy factories increasing your endurance.
ADHD medication also helps. Exercising while there’s more active dopamine in your body is easier.



Wow, there are just so many insights here even if it’s not Proven science, many things resonate with me And I work with alternative Medicine as well, so I’m not glued to modern science, which is extremely limited.
The body is so complex and there are just so many correlating factors. I’ve had low iron for a while and going through perimenopause. It’s so difficult to identify where fatigue can come from what is imagined resistance and what is true resistance from the cell energy perspective, but this is a new perspective for me. Also, as hard as it sometimes feels to start, doing Sports is the biggest ADHD hack I have found so far.
